The various types of heart diseases in children are as follows:-

Aortic Valve Stenosis

This type of pediatric heart disease is congenital. It occurs when the valves of the heart are narrowed. Because of this, the narrowed valve results in a reduced amount of blood being pumped to the body via the artery. The valve here does not open completely.

Aortic Valve Stenosis

Aortic valve stenosis is characterised by chest pain, abnormal heart beat, difficulty breathing, reduced appetite, inability to gain weight etc. Extreme fatigue and dizziness follows when the child is doing some strenuous activity. The child may also have lips and skin discoloration which specify lower oxygen level in the body.

This condition is diagnosed by tests such as ECG, chest x-ray, echo (echocardiogram), and physical tests that include exercise. Following this, the paediatric cardiac surgeon will evaluate the symptoms and see if they increase with time. If they do, then surgery is recommended.

Ebstein’s Anomaly

This paediatric cardiac condition is congenital and begins when one of the baby’s heart valves does not develop completely while it is in the womb. The valve on the right of the heart, which connects the atria to the right ventricle, grows abnormally. This results in two kinds of problems. In one, the valve starts to leak. In the second, the left ventricle is unable to function properly.

The symptoms of this are similar to the symptoms of aortic valve stenosis. These include symptoms such as bluish coloration of lips and skin, fatigue, breathlessness, irregular heartbeat etc.

The tests required to diagnose ebstein’s anomaly are chest x-ray, echocardiogram, oxygen saturation test etc. Usually this is detected by a very large heart that becomes evident in a chest x-ray.

If there are no symptoms, then no medical attention is necessary. However, if there are mild symptoms, then medications are required. Surgery is required when the symptoms are very severe. The surgery for this takes place usually when the child is just born.

Pulmonary Valve Stenosis

This condition begins from the first eight weeks of pregnancy. It is a congenital condition in which the pulmonary valve is not developed properly. This condition is symptomized by similar symptoms as other heart defects aforementioned.

It is diagnosed by a physical examination and the child’s past medical history. Some tests such as chest x-ray, ECG, echo, and cardiac catheterization are also suggested.

After the diagnosis, treatment ranges from requiring no medical intervention to surgery. In conditions where the symptoms are mild no medical intervention is required. In moderate and severe cases, surgical intervention is required.

There are other heart conditions as well. These may be septal defects where  the walls in between the four valves of the heart are  abnormal. There are two further subcategories of this, atrial septal defects and ventricular septal defects.

Paediatric Cardiac Surgery Procedure

Aortic Valve Stenosis

The surgical intervention in children suffering from aortic valve stenosis are of various types. The surgeon decides which ones to use.

 Usually they are subcategories into four types, balloon aortic valvuloplasty, surgical aortic valvotomy, aortic valve replacement, and pulmonary autograft. Except the first one, the rest three involve a surgical procedure.

During the surgery, the surgeon makes the incisions. Following this the surgeon will either remove tissues from the valve’s leaflets ( in the aortic valvotomy procedure), replace the diseased aortic valve with new ones (in the aortic valve replacement), or replace the aortic valve alongside a portion of the aorta with new one ( in the pulmonary autograft surgery).

Pulmonary Valve Stenosis

This surgery takes a few hours. In this surgery, the surgeon makes the incisions and accesses the heart.

In the next step, the surgeon segregates the breast bone. During this time, the patient is attached to a heart-lung machine that does the functions of the heart and lungs while the surgeon operates.

If the pulmonary valves are to be replaced, the surgeon replaces them with a donor pulmonary valve or artificial ones. This procedure is known as valvectomy. Another procedure for pulmonary valve stenosis is pulmonary valvotomy. In this, the surgeon makes an incision in the valve and makes required repairs.

The surgery ends with the surgeon placing the breastbone back together and sealing up the stitches.

Aortic Valve Stenosis Recovery

While recovering from any of the aortic valve stenosis surgery, the child may wake up with pain and soreness. In addition to this, the child’s vital organs such as the heartbeat and blood pressure may be monitored. In most surgeries, the child is required to stay in the hospital for two-three weeks.

The child should under no circumstances do any activities that require strain on the chest. Normal activities should be resumed only after the child been cleared by the surgeon.
