Eligibility criteria for breast reduction surgery

Breast reduction surgery is a major procedure that involves the removal of extra breast fats, glandular tissue, and skin from breasts. Breast reduction surgery’s main aim is to reduce overall breast size and make it proportionate to one’s body size.

Additionally, it also alleviates problems caused by large breasts such as pain in the neck, back, and shoulder.

Symptoms that require breast reduction surgery

  • Severe back, neck, and shoulder pain.
  • Severe rashes and irritation under the breast skin.
  • Limited activity resulting from the weight of the breasts.
  • Poor self-image.
  • Nerve pain.
  • Difficulty breathing.
  •  Appearance of strap marks on the shoulders and around the back.
  • Poor posture and numbness rising upwards from the chest.
  • Gynecomastia, or enlarged breasts in men.

In order to opt for breast reduction surgery one does not need to consult the doctor. The severity of the symptoms hampering daily life activities, while also lowering self esteem, is enough to consider breast reduction surgery. However, there are few conditions in which breast reduction surgery is not possible. These conditions are:-

  • Severe obesity
  • Severe diabetes and heart problems.
  • Family history of clotting disorders.
  • Family history of wound healing disorders.
  • Smoking

Before the surgery, one should inform the doctor about other pre-existing ailments and prior breast surgeries. The surgeon may ask the patient about his/her family history, inquire if there’d been any lump removal surgeries before, and ask about existing eating habits and medications.

Tests required before breast reduction surgery

It is usually recommended that the patient speak to the doctor about the mental and physical impact of having the surgery done.

The doctor generally recommends one to quit smoking and certain medications. Medicines which thin the blood or are aimed at reducing pain, are suggested to be stopped. Alongside this, there are few tests to be conducted. They are:-

  • Blood tests
  • ECG (electrocardiogram)
  • CBC (Complete blood count) test.
  • Urinary pregnancy test.
  • Mammogram, especially for patients in their late forties.

The aforementioned tests are recommended alongside a complete evaluation of  medical history and overall health.

The doctor then performs certain physical evaluations to determine the degree of reduction of breast size. The evaluations include:-

  • Measuring the height and width of breast area.
  • Examining breast tissue distribution.
  • Measuring the surrounding chest and rib cage.
  • Looking for cysts and lumps.

Before breast reduction surgery

In the last consultation, the doctor discusses certain things such as what to expect after the surgery, the technique that will be used in the surgery, the pros and cons of the surgery- such as enhanced quality of life and the loss of sensation.

Your doctor may also suggest some alternatives depending on your evaluation results. If breast reduction surgery is finalized, you must have a clear understanding of what to expect from the doctor. For this, you could ask certain questions such as:-

  • The doctor’s official certification and years of experience in the field.
  • The doctor’s breast reduction’s before-and-after pictures.
  • Possible complications after surgery.
  • Degree of scar visibility.
  • Possible risks post surgery.
  • Steps to be taken in case the results after the surgery does not meet the agreed upon goals.

In some cases for women where family planning is involved, it may be wise to postpone breast reduction surgery. This is because the breast-feeding ability is negatively impacted after breast reduction surgery.

Breast reduction surgery procedure

Breast reduction surgery usually takes three- five hours. During this time, the patient is given general anesthesia. In some cases, the doctor may use additional medicines through the IV.

Step 1

In the first step, the doctor makes incisions using a surgical marker. It is initiated in the nipple area and is continued through a vertical line till the breast crease. Another cut is made below the breast crease.

The incisions made depend entirely on the size of the breasts, the sagging nature of the breasts, and positioning of the nipple-areola. For instance, micro incisions are made in the case of large fatty breasts that do not sag.

Step 2

In the second step, the doctor then carefully removes the excessive fats and breast skin with a scalpel. Liposuction may be used by the doctor in addition to the surgery, for the removal of some extra tissues. Sometimes the doctor may also remove some extra tissues from one breast if they are in improper alignment.

Step 3

In the third step, the remaining skin and fats are reshaped and uplifted. The nipple-areola complex is moved up to a higher position in case of a large breast.

Step 4

The fourth step involves adding extra stitches inside the breast tissue for extra support. The incisions are then closed off and stitches are bandaged.

The patient is either discharged on the same day or kept for two days in order to observe the surgical response.

Recovery after breast reduction surgery

Immediately after the surgery, the patient is removed to the observation area. He or she may find compression sleeves attached to the legs to help with circulation and a drainage tube attached to the breasts to stop fluid build-up.

During this period, the surgical site could become numb. Furthermore, as soon as the anesthesia wears off, the patient may feel some discomfort and pain. If there are no complications, the doctor discharges the patient on the same day. However, in most cases, patients are kept for up to two days to ensure that the surgery is successful.

Before discharge, the patient is given clear instructions on how to take care of the wounds, how to wear compression garments, what kind of diets to follow, and when to return for consultation.

Special instructions with regards to bathing and sleeping are given. These involve suggestions such as sleeping with pillows placed under the upper back and head, or taking sponge baths without wetting the hair.

Within the first week of surgery, the drains and stitches are removed. The patient is advised to refrain from any kind of physical activity for at least a month. However, light walking is recommended to avoid blood clots.

Within the first two-three weeks, few changes become visible in the breast area. This is general and involves swollen and/or bruised breasts with a tender and sensitive feel in the surgical area.

It takes a total of one month to recover. But, since recovery is subjective and dependent on the patient’s response. Usually, the women patients are advised to avoid wearing wired bras and start wearing stretchable elastic bras for a few months.

Risks involved in breast reduction surgery

As with other surgical procedures, there are risks in breast reduction surgery as well. Mentioned below are the risks common to only breast surgery. They are:-

  • Allergic reaction to surgical materials used.
  • Allergic reaction to anesthesia.
  • Hematoma or excessive bleeding. This usually occurs when the patient has been on blood-thinning agents, such as alcohol consumption, immediately prior to the surgery.
  • Surgical site infection.
  • Blood clots or deep vein thrombosis. This results from immobility after surgery. Sometimes the blood-clots may travel up to the lungs or the heart and cause serious damage.
  • Damage of breast nerves, blood vessels, and tissues. Thus usually occurs during the surgery.
  • Numbness in the post-surgical state that could become permanent.
  • Breast asymmetry as a result of inappropriate implant size.
  • Breast lift dislocation, rapture, and deflation.
  • Reduced milk production resulting in difficulty to breast-feed

These are only a few of the risks involved with breast reduction surgery. Only the doctor will be able to provide a complete list of the risks as some of them would entirely be dependent on the patient’s lifestyle and overall health.
